  • The community pharmacist is uniquely situated to provide sustained continuity for patients with or at risk for heart disease. Pharmacists must continue their own education regarding coronary heart disease, so that they can educate and support patients between physician appointments, identify patients needing medical attention, and serve as a community resource for promoting a healthy lifestyle.
